Please only provide information that is relevant to your volunteering. You do not need to provide your full medical history or diagnosis.
Examples may include mobility requirements, accessibility requirements or an adjustment that would help you carry out a volunteering activity safely.
Why do we verify volunteer identity? To help us maintain a safe and well-managed environment for devotees, volunteers and everyone using the temple, CSPT asks registered volunteers to provide photographic identification for identity verification. Your identification will only be accessible to authorised persons responsible for volunteer administration and verification. Once verification has been completed, the uploaded copy will be securely deleted in accordance with our Volunteer Privacy Notice. We will retain a record confirming that identity verification was completed.
